Continuing your care

Follow-up appointments are where we review how your treatment is working, make any necessary adjustments, and plan the next steps. Because Dr David Crocker sees every patient personally, your reviews are always with the same psychiatrist who carried out your assessment or took over your care. You do not need to repeat your history, and nothing gets lost between clinicians.

What follow-up appointments cover

Each appointment is a focused 30-minute consultation by secure video link. The content depends on where you are in your treatment, but a typical review will cover:

• How you have been since we last spoke, including symptom changes, side effects, sleep, mood, and day-to-day functioning.
• A review of your current medication, including whether the dose is right, whether the formulation suits you, and whether any changes are needed.
• Monitoring of physical health parameters such as blood pressure, pulse, and weight, in line with national prescribing guidelines. Your psychiatrist will let you know what to have ready before each appointment.
• Risk and wellbeing, addressed proportionately and without unnecessary repetition.
• A clear plan for what happens next, including when your next appointment should be and any actions for you or your GP.

After each appointment, a summary letter is sent to you and, with your consent, to your GP, so that everyone involved in your care is kept up to date.

ADHD medication titration

If you have recently been diagnosed with ADHD and are starting medication, follow-up appointments are typically every three to four weeks during the titration period while we find the right medication and dose. This process usually takes between three and nine months. Once you are stable, reviews move to every six months. This is the maximum interval for controlled medications, and no further prescriptions can be issued without a review. At that point, Dr Crocker will write to your GP to request shared care so that they can take over routine prescribing.

Other conditions

Follow-up appointments are not limited to ADHD. If we are treating depression, anxiety, OCD or another condition alongside your ADHD or autism, the same structured approach applies. The frequency of appointments is tailored to your clinical needs and adjusted as your treatment progresses.

Fees

Follow-up appointment (30 minutes): £130.

Prescriptions: £30 per prescription for controlled medications (ADHD stimulants such as methylphenidate and lisdexamfetamine), or £25 for non-controlled medications such as antidepressants and mood stabilisers. Issued following your consultation. The cost of the medication itself is paid directly to your dispensing pharmacy and varies depending on the drug and dose.
